Registration packets were mailed in July to members of NAHSL, the NY/NJ Chapter and state medical library associations in New England. If you did not receive a packet or lost any portion of it, choose one of the two options:
- Print the Conference Registration Form (regform2001.pdf)
and mail it, with your check, to:

Questions about conference registration?
- Answers to many questions can be found below. Contact Carolyn Wilcox at cswilcox1@home.com with any other questions.
How can I get a receipt for my registration?
Your check must accompany the registration, and will be cashed when it is received. Receipts will be available at the conference registration desk, or you may fax a request to Marj Anderson, NAHSL Treasurer, at (207) 879-3929 before the meeting. If your institution is writing the check for your registration, please mail a copy of the registration form in case your institution does not include it. Indicate on the duplicate who will be issuing the check.

NAHSL's Federal ID Number is 22-2589873
Cancellations
You may cancel before October 1, 2001 without penalty. After October 1, the NAHSL treasurer and the conference committee will decide whether to charge you the $50 cancellation fee depending on circumstances, particularly the number of meals charged by the hotel.
Your affiliation
Current members of NAHSL and NY & NJ organizations may register as members. NAHSL members may check the web site at http://www.nahsl.org/name2001.html l to check your NAHSL membership status. A special student rate applies for attendees who are not employed full time. Non-members include lapsed members, former members, spouses or children of members, exhibitors beyond the two per organization and other guests.
Registration at the conference
You may register during the conference, but no meals can be provided for walk-in attendees.. The registration desk will be open during times listed below, but only when there is no program scheduled. Yes, you may attend a program while the desk is closed and pay at the earliest available time.

Saturday, October 13 --- 3:30pm-6:30pm
Sunday, October 14 --- 7:00am - 5:00pm
Monday, October 15 --- 7:00am-5:00pm
Tuesday, October 16 --- 7:00am-9:00am
Continuing Education
Registration for the conference is not required for CE attendance. The opening reception on the same day as the CE classes is not included in CE registration.

Confirmation
If you have printed your email address on the registration form legibly, the registrar will send you a generic email acknowledging receipt. Continuing Education registration confirmations will be handled by the Janet Crager, CE Committee Chair. Contact her at Janet_Crager@brown.edu
Exhibitors and Presenters
Everyone must register for the conference. Presenters will be registered by the program committee. Exhibitors will be provided with two full registrations including meals, and will be registered by the exhibits committee. Exhibitors should mail questions to Jill Livingston at jlivingston@lib.uconn.edu.

Choices for the Monday night banquet are included on the "Events" portion of the Website.
